THE INFORMATION THAT IS COLLECTED
When you book an appointment personal details such as your name, email and telephone number are collected. This information is used to identify you and contact you about the appointments and services you have booked. To provide a safe and effective massage treatment we also need to collect information such as your medical background and lifestyle choices. This information is only used to make sure your treatment is as effective as possible. Whilst you use the website we also receive information about your computer such as your IP address, operating system and browser details. This information helps us provide a better website experience for you. When you buy a gift voucher personal details such as your name, email, recipient’s name and possibly a postal address are collected. This information is used to identify you, possibly the recipient of the voucher and provide the service you have requested.

CONSENT
When you provide your personal information in the booking of an appointment, by completing your massage consultation form, making a payment or contacting Makai Massage about the services provided, you are giving your consent to us collecting that information and using it for that specific reason. We will not use your personal information for any secondary reason, like marketing, unless we have asked you directly for consent to do so. How do I withdraw my consent? For the purpose of legal protection Makai Massage is required to hold the personal information you have given to provide you with massage services and the notes about those treatments for a minimum of seven years. Seven years after your last treatment we will permanently delete all your personal information that Makai Massage holds. If you withdraw your consent during the seven year retention period, your data will be archived until the seven year period expires. Whilst your information is archived, Makai Massage will not access or process it in any way except if needed for legal protection or if required to do so by law. Should you wish to withdraw your consent at any time please email chloe@makaimassage.co.uk with your request. How can I access, update or amend my personal information? You have the right to review the personal information that is stored about you. These will be sent by email, in a Microsoft Word document attachment. You also have the right to request an update or amendment to your data if it is incorrect. To action any of these rights at any time please email chloe@makaimassage.co.uk with your request.

DISCLOSURE
Unless required to do so by law, Makai Massage will not otherwise share, sell or distribute any of the information you provide to us without your consent.

DATA HANDLING & STORAGE
The appointment booking service is provided by Setmore. Setmore specialises in providing secure collection, processing and storage of personal data for healthcare practitioners worldwide. Your data is stored through Setmore using their data storage facilities, databases and the Setmore web framework, on secure servers is always encrypted when in storage and whilst being transmitted across the internet. Third Party Services In general, the third-party providers we use will only collect, use and disclose your information to the extent necessary to allow them to perform the services they provide to us. However, certain third-party service providers, such as payment gateways and other payment transaction processors, have their own privacy policies in respect to the information we are required to provide to them for your purchase-related transactions. For these providers, we recommend that you read their privacy policies so you can understand the manner in which your personal information will be handled by these providers. In particular, remember that certain providers may be located in or have facilities that are located in a different jurisdiction than either you or us. If you elect to proceed with a transaction that involves the services of a third-party service provider, then your information may become subject to the laws of the jurisdiction(s) in which that service provider or its facilities are located. To protect your personal information, Makai Massage takes reasonable precautions and follows the industry’s best practices to make sure it is not inappropriately lost, misused, accessed, disclosed, altered or destroyed.

CHANGES TO THIS POLICY
Makai Massage reserves the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take effect immediately upon their posting on the website. If Makai Massage chooses to make material changes to this policy, Makai Massage will notify you here that it has been updated, so that you are aware of what information is collected, how it is used and under what circumstances, if any, is is disclosed. If this business is acquired or merged with another business, your information may be transferred to the new owners so that they may continue to provide you with the massage services you have requested.

APPOINTMENTS
You can make an appointment with Makai Massage in a number of ways including via the website, telephone, text or email. Appointments are available for a variety of durations please specify your preferred duration during enquiry or booking. Upon making a booking you will receive an email confirmation which includes all the important details of your appointment and you will also receive a text 24 hours before your appointment as a gentle reminder. If this is your first appointment with Makai Massage, you will need to fill out a New Client Consultation Form before your appointment can go ahead so please allow time for this if coming for treatment at Hands On Clinic. If Makai Massage is coming to you, then time will be allocated for this during your treatment.

GIFT VOUCHERS
Any gift vouchers purchased from Makai Massage are valid for one year, may not be used passed the expiry date and are non-refundable.

CANCELLATION POLICY
24 hours. Makai Massage requires a minimum of 24 hours notice to cancel an appointment. Failure to attend an appointment with less than 24 hours notice of cancellation will incur the charge for the full appointment.

Please cancel as soon as possible if a foreseeable event is likely to prevent you from attending, for example, illness or adverse weather conditions. If you need to cancel your appointment at short notice then Makai Massage shall discuss the cost implications on a case by case basis. An invoice for any cancellation / ‘no show’ charges will be sent to you for payment by bank transfer within 7 days. Following a cancellation, payment must be received before any further massage bookings can be taken. Late Arrivals: It is important that the agreed appointment time and duration is adhered to. Makai Massage appreciates that sometimes, things happen, and you may arrive a little later or need to leave a bit earlier than planned. This is fine, but Makai Massage cannot make up the missed time by overrunning or adding time to subsequent appointments.

CONFIDENTIALITY
All client information and clinical records will be safeguarded by Makai Massage and will remain confidential. Unless required to do so by law, Makai Massage will not otherwise share, sell or distribute any of the information you provide without your consent. Makai Massage will send you an email after your first appointment containing relevant home care guidelines if consent has been received from you, the client to do so. Consultations and client treatment details will not be discussed with anyone other than the client, unless the client is under the age of 18 or has a care worker or guardian or written consent has been received from the client for information to be shared with other healthcare practioners to ensure the client is provided with the most appropriate treatment. In order to prevent unauthorised access or disclosure, security measures are employed to protect your information from access by unauthorised persons and against unlawful processing, accidental loss, destruction and damage. Suitable physical, electronic and managerial procedures are in place to safeguard and secure the information collected online. Makai Massage will retain your information for a reasonable period or as long as the law requires. Makai Massage will have access to these records solely for the purpose of providing you with the most appropriate treatment.

HEALTH & SAFETY
Chloe at Makai Massage is a qualified therapist and holds professional and public indemnity insurance for the services provided. Due to the nature of massage and the need for close contact, Makai Massage will maintain a high standard of personal hygiene and clients are asked to ensure that they also maintain a high standard of personal hygiene. If Makai Massage is unwell, suffering from a cold or virus, infection or general ill health, they will not work due to the risks of cross contamination. This may result in Makai Massage contacting you to inform you of the issue at the earliest opportunity to make suitable alternative arrangements. In this instance refunds will not be issued but appointments will be rescheduled. Clients are asked not to attend for a massage if they are also unwell, suffering from a cold or virus, infection or general ill health. Please contact Makai Massage at the earliest opportunity to rearrange your appointment, refunds will not be issued in this instance but appointments can be rescheduled. If you are unwell and contact us to inform us of your illness, please note that our Cancellation Policy may still apply. Makai Massage will not be held responsible for the loss or damage of personal items during your time with Makai Massage. Clients under the age of 16 years must be accompanied by a parent or guardian. This parent or guardian must give permission for the agreed massage plan to be performed, and will be requested to co-sign the New Client Consultation Form. They will be invited to stay in the room for the duration of the appointment, and expected to remain present throughout the treatment. Makai Massage holds the right to refuse any treatment if a client is physically or verbally abusive, or demonstrates inappropriate, aggressive or sexual behaviour. Such behaviour may result in a partial or total ban from treatments with Makai Massage and may also result in criminal proceedings.

DISCLAIMER
Makai Massage provides massage therapy in various forms but none of these bodywork modalities are a substitute for professional medical care and/or counselling. Makai Massage will not diagnose, prescribe medication or medical treatment of any kind and will always recommend referral to a GP or other relevant qualified medical professional, specialist or practitioner.

You as the client have a duty to provide accurate and true medical and personal information, to the best of your knowledge, and to keep Makai Massage up to date with details of any medication, symptoms, medical concerns or treatments you are having investigated or undergoing treatment for.

Makai Massage may refuse to treat any client with just and reasonable cause. Makai Massage reserves the right to terminate any appointment at any time if they believe that it is not safe or in the client’s best interest to continue. Likewise, the client has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.
